In the Identification block, choose the Name Type. If the Name Type is empty, choose WFMO Name PREVName Change. TabSave.

If the Name Type has a value, select the Alternate Identification tab. Insert a new record. The Name Type is PREV (Previous Name). Enter the current name and save.
