When the form is received by the Registrar’s Office, documentation is validated and if appropriate, the name change is completed in SPAIDEN.

  1. In SPAIDEN, enter the student’s ID. Go.

  2. In the Identification block, choose the Name Type. If the Name Type is empty, choose WFMO Name PREVName Change. TabSave.

  3. Overwrite the correct name. Save.

  1. Mark the Frevvo form as complete.

Troubleshooting

If the Name Type has a value, select the Alternate Identification tab. Insert a new record. The Name Type is PREV (Previous Name). Enter the current name and save.
