Changing a student's name
Current students should use the name change form to submit a request to change their name. Former students and alumni should use the PDF.
Students who are current or former employees must submit a name change via Human Resources.

Accessing name change requests
The name change form is a Frevvo form.
When the form is received by the Registrar’s Office, documentation is validated and if appropriate, the name change is completed in SPAIDEN.
Change the name in SPAIDEN
In SPAIDEN, enter the student’s ID. Go.
In the Identification block, choose the Name Type. If the Name Type is empty, choose PREV Name Change. Save.
Overwrite the correct name. Save.
Mark the Frevvo form as complete.
Preferred names cannot include special characters (accents, umlauts, etc.)
Troubleshooting
If the Name Type has a value, select the Alternate Identification tab. Insert a new record. The Name Type is PREV (Previous Name). Enter the current name and save.
Add the new name in the Current Identification tab by selecting Name Type as WFMO Name Change; overwrite the correct name and Save.
If a PREV name type already exists in the Alternate Identification tab, remove the name type from that name. Insert a new record and choose the Name Type as PREV. Only one name can be assigned a given Name Type, so only one name can have the PREV Name Type.
For employees who have changed their name in HCM: When the process runs to update Banner, it's trying to move the name you’re changing from to the Alternate Identification tab but it already exists there so it causes an error and won’t update. Go to the Alternate Identification tab, go to the record with the name you’re changing from and add PREV to the Name Type (it's currently null), then save. This might make it unique and it should update overnight. This is a temporary workaround with a permanent solution currently in development.
